Responsible for the implementation of individualized services in all Day Habilitation areas emphasizing community integration and in-house activities to enhance the People Served in their chosen outcomes affecting his/her capacity to live, work, socialize, and participate within the community.

Essential Duties and Responsibilities

  • Directs services to people served based upon their choice and person-centered planning in an individualized service plan.
  • Implements all daily activities including community integration, in-house activities, skills training, guest speakers, volunteer activities, travel training, adaptive technology, and physical assistance
  • Directs services to people for their physical needs including lifting with moderate to heavy two-person lifts. Feed, reposition, or ambulate as prescribed by a physician or PT consult. Proven competency with lifting using a variety of mechanical lifts.
  • Reports information regarding people's well being to the designated supervisor re advocacy, health, and emotional issues. Communicates any or all issues to the Independent Living Specialist, and supervisors.
  • Responsible for taking attendance, transportation data, daily logs, and meeting documentation. Assist with the accurate recording of persons served financial records.
  • Responsible for the daily documentation of goal implementation meets all requirements as per regulations.
  • Adherence to all rules, regulations, policies, and procedures of Federal, State, and Agency Programs.
  • Assists with busses, including daily driving individuals served in Agency vehicles. This also includes the proper tie-downs of wheelchairs and operation of lifts for loading and unloading.
  • Responsible for maintaining van logbooks, program equipment, Day Hab site, and Agency cleanliness and equipment. May include washing windows, removing snow and steam cleaning interiors.
  • Supports and enhances the People served as the major decision-makers affecting their services.
  • Supports Self-Advocacy.

Daily van driving for community integration.

Education and/or Experience

High school diploma or general education degree (GED); or one to three months related experience and/or training.

Certificates, Licenses, Registrations: Valid & Clean New York State Driver's License

Job Type: Full-time

Salary: $13.00 to $14.30 /hour
